Requesting a Recommendation?

If you would like me to write you a letter of recommendation, please request the letter at least one month in advance of its due date. If I agree to write the letter, please send me an email that answers the following questions and includes your cv/resume and the personal essay that you are submitting with your application.

• What name do you prefer for the letter (ie- Can I use Liz instead of Elizabeth)?
• What is your major?
• What is your year of study?
• What is my relationship to you (instructor, research adviser, etc.)?
• What are you applying for and what are the corresponding application due dates? Tell me about the program(s) and why you are interested. Providing a link to the program(s) would be great.
  
• What are your long term goals and how will this position help you attain those goals?
• Is there anything specific you would like me to address in the letter?
